IADFS CASE OF THE WEEK 2: Diabetic Foot Functional Limb Salvage


71, M, Type 2 DM, moderate PVD. Previous bilateral transmet amputation for infections. Developed progressive deformity and instability left hindfoot with ulceration due to Charcot. Managed with hindfoot reconstruction using a hindfoot nail and additional screw. Ulcer healed and full weight bearing commenced in surgical shoes.


IADFS CASE OF THE WEEK 1: Diabetic Foot Functional Limb Salvage

60, F, very active lady. Type 2 DM. Ankle fracture following minor trauma- clinically a Charcot. Internal fixation failed after commencing weight bearing at 8 weeks. Ankle arthrodesis with screws failed to fuse. Frame fixation failed to fuse. Standard hindfoot nail failed to fuse and developed frank infection. 

Managed with two stage reconstruction. 1st stage of extensive debridement, temporary wire fixation, local antibiotic elution and post-op NPWT to the wound. After infection eradication, the second stage with internal fixation and local antibiotic elution. Full bone fusion and functional limb salvage achieved.

Anatomical and Surgical Dissection of the Foot - The Reconstruction of the Diabetic Foot

The Association of Diabetic Foot Surgeons (ADFS) is organising this year a cadaver course in Padova (Italy) in cooperation with the Plastic Surgery Department and Human Anatomy Institute of the Padova University School of Medicine.
